Instructions for Submitting a Claim

  1. Complete the electronic form on this page. Required fields will be highlighted in red.
  2. Click "SUBMIT".
  3. Select the email provider from which you'd like to send your completed form.
  4. Attach required proof of loss and value. Proof of loss should include original commercial invoice, packing list and signed delivery receipt noting exceptions.
    • For damage claims, please include photos of both the damaged packaging and its contents.
  5. Send the email.
